Telehealth ROCKS Communities

Focus:

Address social determinants of health by the integration of school, clinical, social services and community.

About:

Telehealth ROCKS Communities aims to link children and families across systems of care through the use of Community Health Workers in schools. 

Funding:

This program is funded by HRSA through the Evidence‐Based Tele‐Behavioral Health Network Program Grant # G01RH42541. 

Components:

Specialty behavioral health and other clinical services.

Community Health Workers in Pittsburg and Coffeyville Schools.

Community Care Link Software for social determinants, screening and social service navigation.

Clinical, community and social service partnerships to create a robust and comprehensive network.

Goals:

  • Increase access to health care services via direct-to-consumer in rural southeast Kansas in partnership with existing providers in the region. 
  • Contribute to the Evidence Base assessing the effectiveness of direct-to-consumer behavioral health care services along with social services assistance.
  • Effectively manage the project to increase access to behavioral health and social services in southeast Kansas.
